Emma Hayes' team fell behind to a first-half goal from West Ham's Yui Hasegawa, but Drew Spence equalized in the second half and Guro Reiten scored an extra-time penalty to seal the win for the visitors.
The victory moved Chelsea back to the top of the WSL table, one point ahead of Arsenal, who also won their match on Sunday.
